- Recommendations for next steps
After an initial consultation, employees can "buy-up" to a more comprehensive level of service such as WeCare+ Enhanced or WeCare+ Premium.
WeCare+ Enhanced Includes:
- Comprehensive, in-home needs assessment by an RN
- Individualized care plan
- Resource and referral information
- Help negotiating discounts with providers
- Initial care coordination
- Access to LTS Online Knowledge and Education Center
- Access to WeCare+ Talk secure and interactive web portal
WeCare+ Premium Includes:
- Comprehensive, in-home needs assessment by an RN
- Individualized care plan
- Resource and referral information
- Help negotiating discounts with providers
- Initial and ongoing care coordination
- Follow-up and care monitoring by LTS Care Coordinator for the duration of the subscription (1 year)
- Assistance with care plan and service changes
- Access to LTS Online Knowledge and Education Center
- Access to WeCare+ Talk secure and interactive web portal
